TITLE: Hospice Admissions Visit Nurse (RN)
OFFICE LOCATION: Alameda
PATIENT TERRITORY: Fremont - Oakland
SCHEDULE: Per Diem (16 Hours/Week)
Required Shifts: 10am-6pm (M-F, 1 Weekend per month, and potential Holidays)
The posted compensation range of $56.95 - $78.30/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.
POSITION SUMMARY: The Hospice Admissions Visit Nurse is responsible for evaluating and admitting patients to Hospice; for documenting observations and interventions related to the patient’s environment, physical, mental, emotional and social status; and for the coordinating care with identified primary Case Manager and designated Clinical Manager as appropriate.
AREAS OF RESPONSIBILITY:
- Admits patients to service. Assesses patient’s environment; physical, mental and social status and eligibility for hospice services in accordance with hospice admission requirements.
- Assures completion of all documentation including the plan of care
- Assures that all applicable services are ordered, explained, and offered. Obtains physician orders for services, equipment, medication as needed.
- Assesses need for and coordinates services of all other IDG member, including Physician, Social Worker, Home Health Aide, Dietician, Volunteer, and Spiritual Care.
- Documents observations, clinical findings, problems, skilled interventions, goals and plans in a timely manner (including documenting appropriately on Point of Care/laptop device).
- Makes skilled nursing revisits to established patients as directed by Patient Care Manager.
- Provides and documents skilled nursing care (includes skilled observation of the patient’s condition, skilled care and procedures and teaching of the patient and/or family), on all intermittent visits and/or shifts.
- Provides supervisory visits for the Home Health Aides as directed and as they spontaneously occur.
- Demonstrates all skills of a registered nurse, including those skills required by the agency through competency testing.
- Maintains and updates own level of knowledge and skills required to provide quality hospice nursing care.
- Participates in QA / PI activities and in-service education related to areas of practice as appropriate.
- Assists in development of hospice protocols, procedures and policies as requested.
- Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways.
QUALIFICATIONS:
- California RN license required: PHN preferred.
- Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing.
- Must be highly motivated, self directed, flexible, and able to work well and participate in a multidisciplinary team setting.
- Current California driver’s license and automobile insurance.
- Current CPR certification.
