TITLE: Spiritual Care Manager (Chaplain Manager)
SCHEDULE: M-F
HOURS: 8:30am-5pm

The posted compensation range of $36.83 - $50.63/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.
POSITION SUMMARY:
The Spiritual Care Manager is responsible for the management and coordination of the Hospice spiritual care program, for overseeing the provision of spiritual counseling services to patients and families when requested and for psycho-spiritual support services to Hospice staff.
AREAS OF RESPONSIBILITY:
- Assures the development and implementation of a spiritual plan of care for patient/family with the interdisciplinary team.
- Hires, orients, trains, supervises and evaluates assigned staff.
- Facilitates and assures spiritual counseling services quality of care provided to meet needs of patient/family.
- Coordinates and supervises the process to ensure the optimal level of Spiritual Care services are provided to Hospice and Home Health patient and Families.
- Develops community spiritual care resources and contacts with clergy or counselors who provide care to Hospice/Home Health patients.
- Oversees interdisciplinary team meetings for optimal collaboration and coordination for Spiritual Care providers with other patient care providers.
- Oversees and (for a small case load) provides counseling to assist patients/families to integrate the dying experience into their lives and to find meaning and purpose in what remains of life.
- Ensures Pt./Family requests for memorial/funeral services support is appropriately provided.
- Facilitates commemorative rituals for Hospice staff and patients' family members as appropriate to need.
- Facilitates activities such as retreats, regular support group meetings, individual support that assist staff with their skills in providing staff, Pt./Families with Spiritual Care support.
- Provide training, educational programs and workshops for specifically for Spiritual Care Staff, but also as appropriate for Hospice staff, families and the community (as appropriate).
- Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways. This position in particular will oversee:
- 1. The Home Health connection for Spiritual Care providers.
- 2. The Pediatric Team Spiritual care support.
- 3. Oversee the volunteer and the development for the Transitions Program.
QUALIFICATIONS:
- MDiv or equivalent graduate degree from a theological program, or a BA/BS and a combination of experience/training in spiritual care.
- Certified chaplain
- Previous Management Experience Preferred
- Experience in Hospice or Palliative care.
- Demonstrated leadership skills and experience in the development of collaborative relationships with others; management/supervisory experience preferred.
- Ability to work independently and creatively in order to accomplish goals with a minimum of supervision.
- Excellent verbal and written communication skills.
- Ability to work collaboratively and respectfully with persons following traditional and non- traditional spiritual practices.
- Ability and willingness to work a flexible work schedule including evenings and weekends as needed.
- Valid California Driver's license and current automobile insurance.
- Current CPR certification.
